Yes, you can put an outlet in the cabinet (consider putting in multiples/plugmold so you can use it as a charging station for all your small electronics and phone type thingies). You can also wire your kitchen for speakers before you put in the cabinetry. There are also wireless speakers. My speakers are in the ceiling.

You say it's not in the NEC, I'll take your word for it- saying "keep looking" is not saying "you're illiterate" just well, keep looking. You never know. Getting back to question 1, speakers, unless they're electrostatic, work at a pretty low voltage and are more or less isolated pretty effectively, through the way they're wired and how they go through the amp. In the absence of a ruling, and given that outdoor speakers exist, I doubt they will be a concern...if you've examined the code and found no prohibition, I'd say that was ok. If in doubt, ensure the amplifier, which will after all, be the item connected to the mains current, is on a GFCI. If you were to do that, you'd be covered anyway. Personally, I think all household outlets ought to be on them. Did I not answer question two adequately? Excccccuuuuusssse me. Moisture WILL ruin a conventional speaker. Most are, after all, made from a form of paper....not a good combination with damp air. If you were to opt for the in-ceiling insert type, you will need to ensure the moisture barrier is intact or repaired - you know, one of those enclosure things, I guess, to avoid steam from the bathroom entering the ceiling cavity. For those reasons, non-recessed speakers might be better, I've seen bracket-mounted ones for boats etc, they'd probably be ideal. If you opt for the recessed type, you should consult the building code, separate from the NEC as you'll be (potentially) breaching the moisture barrier. Lastly, I don't know how the NEC defines 'damp' and 'wet' but I can 1) hazard a good GUESS which is the ONLY ANSWER you've gotten thus far, that you might as well take as READ that it'd be a WET location. A shower is a WET location, whether the shower head is a hand-held one or wall mounted, if I understand that's what you're proposing, well, duh, it's a shower. And, uh, you've got a shower with in the same room, well, it's a wet location. I do understand there are regulations in particular to things directly above showers, but in your case with very little extra expense, you can be covered anyway. Point 2, is I thought it safe to venture an opinion without CONCRETE knowledge of the rule, since you will get into NO trouble, by over-speccing, whereas if you go under, you may. Treat it as a wet location, GFCI the stereo equipment, and I'd confidently say, you're covered by code.....unless you're thinking the speaker wires themselves ought to be protected separately, however, I don't see how they could. I felt the same way with my bed. I also have windows that flank the bed on either side but mine are a bit farther away than yours. Although I have a lot of framed art I could have put above it, I didn't want to take away from my pretty headboard. I also felt that the shape of a straight-edged picture frame or a round/oval mirror would work against the scrolled shape of my headboard. It may not be your choice, I found a simple cross that I've hung there and love the look. The ceiling is high enough that I felt I did need something there, but it didn't have to fill the space. This is why I took pains-at the very outset-to stress the point that it is a matter of degree. In no way would a light coat of pea gravel-or any other course aggregate-cause trouble. And as Dave in NoVa rightly asks, how many older trees have no shade of their own root zone? Not many, I'd warrant. The simple answer is, this should not be a problem. I guess if we want to split hairs endlessly, then yes, we can endlessly dream up ways in which it would cause great harm. But that would all be BS.
